A waiver must be completed in full for each person participating. *Under 18 must have parent sign.
PRACTICE BILLING / CANCELLATION / REFUND POLICY
A credit card is required to be on file with our office in order to practice in our facility.
We prefer payment in full prior to the start of your practice and at the latest the end of the practice. We will allow leniency should you not have total payment, however the card on file with our office will be charged the next day for the remaining balance for that practice.
You have 48 hours to contact our office and cancel your practice without penalty. Cancellations made in less than 48 hours advance notice will result in a 50% cancellation fee which will automatically be charged to the card on file.
In event of a “no-show” your card will be charged the full cost.
